Health Forms for Your Child’s Wellbeing

Health is a critical component of education. To ensure your child gets the best support, we require the completion of specific health forms. These include the Medication Permission Form, Physical Exam Form, and Private Dental Exam Form. Filling out these documents enables us to accommodate your child's health needs effectively while they are at school.

Tips for Creating PDF Files

Many of Wyncote Academy’s online forms require uploading documents that are in PDF format. There are many ways to create PDF files using your smartphone’s camera, but it isn’t just “taking a picture”. The following tools can be used to create PDF files and are free or have free versions available from whatever app store you use.
